Lipomatous metaplasia: a poorly recognized entity of myocardial infarction

Lipomatous metaplasia of the myocardium is a poorly recognized pathology of chronic ischemic cardiomyopathy due to little knowledge and the fact that it is diagnosed mainly by computed tomography and cardiac resonance (MRI), techniques that are not easily accessible within the hospital setting. Studies have described lipomatous metaplasia as a predictor of hospitalization for heart failure, ventricular arrhythmias, and all-cause mortality in patients with chronic ischemic heart disease. MRI provides us with a better characterization of the myocardium, and T1-weighted sequences with and without fat suppression show high-signal images with better diagnostic efficiency compared to computed tomography.
