A 45-year-old male, with a record of high blood pressure with poor medication adherence, deep vein thrombosis, post-COVID-19 bilateral hypoacusis, tobacco and energy drink consumer, presented with ST-segment elevation myocardial infarction. The coronary angiography revealed simultaneous thrombosis of both coronary arteries, without atherosclerotic disease. In etiology diagnosis, V Leiden factor mutation and protein S deficiency were identified.
