Tricuspid valve infective endocarditis is rare and usually affects people who use intravenous drugs or have intravascular devices. We present the case of a 24-year-old man with infective endocarditis complicated by septic pulmonary embolism. His main risk factor was intravenous drug use. The diagnosis was made according to the European Society of Cardiology 2023 criteria. He received medical and surgical treatment due to the high probability of new embolic events due to the echocardiographic characteristics of the vegetation. Early diagnosis and timely surgical intervention, combined with addiction management, are key to improving outcomes in this uncommon type of endocarditis.
